THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by letting us know in writing.
How We Typically Use or Share Your Health Information
- Treatment. We use and share your information to fill and compound your prescriptions, counsel you, check for drug interactions, and coordinate with your prescribers and other health care providers.
- Payment. We use and share your information to bill and get payment from health plans, pharmacy benefit managers or other payers.
- Health care operations. We use and share your information to run our pharmacy, improve quality, train staff, and contact you when necessary, for example with refill reminders or ready-for-pickup notices.
Other Ways We May Use or Share Your Information
We are allowed or required to share your information in other ways, usually in ways that contribute to the public good, such as public health and research. We have to meet many conditions in the law before we can share your information for these purposes:
- Public health and safety, including reporting adverse drug reactions or product problems to the FDA, preventing disease, and helping with product recalls
- Reporting suspected abuse, neglect or domestic violence, or preventing a serious threat to health or safety
- Prescription drug monitoring programs (such as Florida E-FORCSE) as required by law
- Complying with the law, including when the U.S. Department of Health and Human Services wants to see that we are complying with federal privacy law
- Health oversight agencies, such as the Florida Board of Pharmacy and the Drug Enforcement Administration, for audits, inspections and licensure
- Responding to lawsuits and legal actions, law enforcement requests, workers’ compensation claims, and specialized government functions
- Coroners, medical examiners, funeral directors, and organ and tissue donation organizations
- Business associates who perform services for us (for example, our pharmacy software, telephone and texting providers), who must protect your information under a written agreement
- Family members or others involved in your care or payment for your care, such as someone picking up your prescription, unless you object
Uses That Require Your Written Permission
We will not use or share your information for marketing, sell your information, or share it for other purposes not described in this notice unless you give us written authorization. You may revoke an authorization in writing at any time, except to the extent we have already relied on it.
Your Rights
- Get a copy of your health and claims records. You can ask to see or get an electronic or paper copy of your pharmacy records. We will provide a copy or a summary, usually within 30 days of your request. We may charge a reasonable, cost-based fee.
- Ask us to correct your records. You can ask us to correct information you think is incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
- Request confidential communications. You can ask us to contact you in a specific way (for example, a different phone number or address). We will say yes to all reasonable requests.
- Ask us to limit what we use or share. You can ask us not to use or share certain information. We are not required to agree, except that if you pay for a service or item out of pocket in full, you can ask us not to share that information with your health insurer, and we will say yes unless a law requires us to share it.
- Get a list of those with whom we have shared information. You can ask for a list (accounting) of the times we have shared your information for six years prior to the date you ask, who we shared it with, and why, other than for treatment, payment and operations and certain other disclosures.
- Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. If you have given someone medical power of attorney or someone is your legal guardian, that person can exercise your rights.
- File a complaint if you feel your rights are violated. See below. We will not retaliate against you for filing a complaint.
Communicating With Us by Text, Email or Voicemail
If you choose to contact us by text message, email or voicemail, you understand these methods may not be fully secure. We use service providers that have agreed to protect your information. Please do not send detailed medical information by email; call us instead if you prefer.

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our pharmacy, and on our website.
